Optik Nöropati Gelişmemiş Tiroid İlişkili Oftalmopati Hastalarında Kontrast Duyarlılık Fonksiyonlarının Sağlıklı Popülasyonla Karşılaştırılması

Abstract

Amaç: Bu çalışmada, TİO tanısı konulmuş olan hastaların kontrast duyarlılık seviyelerinin normal sağlıklı bireyler ile karşılaştırılması amaçlanmıştır.Metod: Kliniğimize başvuran 20 ile 65 yaş arası kadın ve ya erkek, graves oftalmopati tanısı ile takipli, görme keskinliği normal veya normale yakın olan 20 hasta ve benzer özelliklerde sağlıklı 20 bireyden oluşan kontrol grubunun kontrast duyarlılık fonksiyonları prospektif olarak değerlendirildi. Göz muayene bulguları kaydedildi. Kontrast duyarlılık fonksiyonları 1.5, 3, 6, 12 ve 18 cpd olan beş frekansta ` Functional Acuity Contrast Test' (F.A.C.T.) ile ölçüldü. Tüm veriler istatistiksel olarak karşılaştırıldı. Sonuçlar hasta ve kontrol grupları karşılaştırmaları ile değerlendirildi.Bulgular: Optik nöropati bulunmayan TİO' li olgularda kontrast duyarlılık fonksiyonu sonuçları kontrol grubu ile karşılaştırıldığında tüm uzaysal frekanslarda istatistiksel olarak anlamlı düşük bulundu (p<0.05). TİO olguları ile ve sağlam olguların sağ ve sol gözlerinin kontrast duyarlılık fonksiyonları karşılaştırıldığında; KDF' nun daha yüksek frekanslarda (B, C, D, E frekansında) daha belirgin olarak etkilenmiş olduğu tespit edildi (p<0,05). Hasta grubunun sağ gözü ile sol gözü arasında ve kontrol grubunun sağ gözü ile sol gözü arasında kontrast duyarlılık fonksiyonu sonuçları karşılaştırıldığında istatistiksel olarak anlamlı bir fark tespit edilmedi (p> 0,05). TİO' li gözlerin hertel ve KAS değerleri ile kontrast duyarlılık fonksiyonu arasında istatistiksel olarak anlamlı korelasyon mevcut değildi (p> 0,05).Sonuç: Otoimmun tiroid hastalığı gözü birçok yönden etkileyen bir hastalıktır. Hastalık progresyonu açısından erken tanı ve tedavinin önemi herkesçe kabul edilen bir faktördür. Bu nedenle, yapılacak ileri çalışmalar TİO' nin tanı, tedavi ve takibinde kontrast duyarlılık testinin etkinliğini belirlemede faydalı olacaktır.Anahtar Kelimeler: Tiroid ilişkili oftalmopati, optik nöropati, kontrast duyarlılık fonksiyonu, F.A.C.T.

Materials and methods: The study includes 20 patients at the age between 20 and 65 years who have been diagnosed as TRO and have a normal or near normal visual acuity. The control group is formed by 20 healthy people who have the same demographic properties. The contrast sensitivity functions of these two groups are evaluated prospectively. Ophthalmic examinations were enrolled. Contrast sensitivity functions were measured by the functional acuity contrast test (F.A.C.T.) in the five frequency which was 1.5, 3, 6, 12, 18 cpd. The data were compared statistically and results were enrolled by the patients and control group confrontation.Results: The results of contrast sensitivity functions of two groups were compared. There were statistically significant low levels in the all frequencies of contrast sensitivity functions in the thyroid related ophthalmopathy group which doesn?t have optic neuropathy (p< 0.05). When the contrast sensitivity functions of the right and the left eyes of the two groups were compared; it?s found that contrast sensitivity functions are more affected in the high frequencies (B, C, D, E ) (p < 0.05). There wasn?t statistically significant difference about the contrast sensitivity functions results of the right and the left eyes in each group and also statistically significant correlation was not found between the contrast sensitivity functions and Hertel measurements and clinic activity scores results (p > 0.05).Conclusion: Autoimmune thyroiditis affects the eyes in more different sides. Early diagnosis and treatment is an important factor for the progression of the disease. Further studies may show that effectiveness of the contrast sensitivity functions test for the diagnosis, treatment and following of the thyroid related ophthalmopathy.Key words: Thyroid related ophthalmopathy, optic neuropathy, contrast sensitivity function, F.A.C.T.
