Publication: Evaluation of the Effect of Root Canal Treatment on the Quality of Life of the Patients
Abstract
Amaç: İlk kez yapılan (primer) ve yenilenen kök kanal tedavisinin (retreatment) hastaların yaşam kaliteleri üzerine etkisini değerlendirme, kısa ve uzun dönem sonuçlarını karşılaştırmaktır. Materyal ve Metot: Çalışmamızda dahil edilme kriterlerini karşılayan 150 hastaya (75 kök kanal tedavisi-75 retreatment) tedavi öncesi, tedavisi sonrası 1. ve 6. ayda ağız sağlığı ile ilişkili yaşam kalitesini değerlendiren ölçeklerden olan Oral Healthy Impact Profile (OHIP-14) anket formu dolduruldu. Sosyodemografik verileri, tedavi öncesi ve tedavi sonrası Visual analog skala (VAS) skorları, Periapikal indeks (PAI) skorları, dişle ilgili endodontik faktörleri içeren bilgiler de anket formuna eklendi. Sonuçların istatistiksel analizi yapıldı. Bulgular: Çalışmamızın sonuçlarına göre kök kanal tedavisi ve retreatment tedavisi ağız sağlığı ile ilişkili yaşam kalitesini (ASYK) artırmaktadır.Yapılan anket sonuçlarına göre 1. ve 6. ayda OHIP-14 skorları anlamlı olarak daha düşük bulundu. Bu da ağız sağlığı ile ilgili yaşam kalitesinin arttığını göstermektedir. Tedavi öncesinde yapılan anket sonuçlarına göre her iki grupta da OHIP-14'ün alt boyutlarından en yüksek skora fiziksel ağrı, sonra en yüksek skora ise psikolojik rahatsızlık boyutu sahiptir. VAS skorları yüksek olan ve PAI˂3 hastaların başlangıç OHIP-14 skorları anlamlı olarak daha yüksek bulundu (p˂0,05). Kadınlarda başlangıç OHIP-14 skorları erkeklere oranla daha yüksek bulundu (p˂0,05).Vital dişleri bulunan hastaların kısa dönem sonuçlarına göre (1ay) yaşam kalitesindeki düzelme oranı anlamlı olarak daha yüksek bulundu (p˂0,05). Vital dişleri olan hastaların nekrotik dişleri bulunan hastalara göre 1. ay sonu skorlarındaki düzelme oranı daha yüksek bulundu. Kök kanal tedavilerinden sonra OHIP-14 ün tüm alt boyutlarında iyileşme görüldü. Sonuç: Bu çalışmanın sonuçlarına göre endodontik tedavi gereksinimi duyan hastaların yaşam kalitelerinin düşük olduğu görüldü. Kök kanal tedavisi ve retreatment tedavisinin ağız sağlığı ili ilişkili yaşam kalitesini artırdığını sonucuna varıldı. Anahtar Kelimeler: Kök kanal tedavisi; Retreatment; OHIP-14; Yaşam kalitesi
Aim: To evaluate and compare the effect of first time and renewed root canal treatment (retreatment) on patients' quality of life. Materials and Methods: In our study, 150 (75 root canal treatment-75 retreatment) patients who met the inclusion criteria were filled in the OHIP-14 questionnaire, which is one of the scales evaluating oral health-related quality of life, before the treatment, at the 1st and 6th months after the treatment. Sociodemographic data, pre-treatment Visual analog scale (VAS) scores, Periapical index (PAI) scores, and information including dental endodontic factors were also added to the questionnaire. Statistical analysis of the results was performed. Results: According to the results of our study, root canal treatment and retreatment increase oral health-related quality of life. According to the results of the questionnaire, OHIP-14 scores were found to be significantly lower in the 1st and 6th months. This shows that the quality of life related to oral health has increased. According to the results of the initial questionnaire, physical pain has the highest score and psychological discomfort dimension has the highest score among the sub-dimensions of OHIP-14 in both groups. Initial OHIP-14 scores of patients with high VAS scores and PAI˂3 were found to be significantly higher (p˂0,05). Female baseline OHIP-14 scores were higher than males (p˂0,05). According to the short-term results (1 month) of patients with vital teeth, the rate of improvement in quality of life was found to be significantly higher (p˂0,05). Conclusion: According to the results of this study, it was observed that the quality of life of the patients who needed endodontic treatment was low.
