Hemodiyaliz Hastalarının Manevi Yönelimleri İle Umutsuzluk Düzeyleri Arasındaki İlişki

Abstract

Amaç: Bu araştırma hemodiyaliz hastalarının manevi yönelimleri ile umutsuzluk düzeyleri arasındaki ilişkiyi belirlemek amacıyla tanımlayıcı olarak yürütülmüştür. Materyal ve Metot: Araştırmanın evrenini 15.07.2017-15.05.2018 tarihleri arasında Ondokuz Mayıs Üniversitesi Sağlık Uygulama ve Araştırma Merkezi Hemodiyaliz Ünitesi, Nefroloji Servisi ve aynı kuruma bağlı Şehir Sağlık Merkezi'nde tedavi gören 100 hemodiyaliz hastası oluşturmuştur. Veriler hasta tanıtıcı bilgi formu, Manevi Yönelim Ölçeği (MYÖ) ve Beck Umutsuzluk Ölçeği (BUÖ) kullanılarak toplanmıştır. Verilerin normal dağılıma uygunluğu Shapiro Wilk testi ile incelenmiştir. Normal dağılıma uymayan verilerin karşılaştırılmasında Kruskal Wallis ve Mann Whitney U testi kullanılmıştır. Ölçeklerin alt boyutları arasındaki ilişkinin incelenmesinde ise Spearman korelasyon analizinden yararlanılmıştır. Bulgular: Hastaların yaş ortalamasının 56,7±14,4 olduğu, % 53'ünün 1-5 yıl önce kronik böbrek yetmezliği tanısı aldığı, % 75'inin 1-5 yıldır hemodiyaliz tedavisi aldığı, % 63' ünün başka bir kronik hastalığı olduğu tespit edilmiştir. Hastaların % 51'i manevi durumunu, % 70'i ise umut durumunu orta düzey olarak bildirdiği belirlenmiştir. Bu araştırmada MYÖ toplam puan ortalaması 99,57±15,51 olarak saptanmıştır. BUÖ toplam puan ortalaması ise 5,68±4,43; gelecek ile ilgili duygular ve beklentiler alt boyutu toplam puan ortalaması 1,2±1,29 iken motivasyon kaybı alt boyutu toplam puan ortalaması 2,56±1,89 ve umut alt boyutu toplam puan ortalaması 1,91±1,82 olarak belirlenmiştir. MYÖ ile BUÖ arasında negatif yönlü zayıf düzeyde anlamlı ilişki belirlenmiştir (p<0,05). Sonuç: Hemodiyaliz hastalarının manevi yönelimleri arttıkça umutsuzluk düzeylerinin azaldığı belirlenmiştir. Aim: This research was carried out as a descriptive study to determine the the relationship between the spiritual orientations of hemodialysis patients and hopelessness levels. Material and Method: The population of the research study consists of 100 hemodialysis patients who were treated at the hemodialysis unit of Ondokuz Mayıs University Health Research and Application Center, nephrology service and the City Health Center of the same institution between 25.06.2017-25.12.2018. The data were collected by using the patient information form, Spiritual Orientation Scale and Beck Hopelessness Scale. The compliance of the data with normal distribution, was examşned by using the Shapiro Wilk test. Kruskal Wallis and Mann Whitney U tests while the relationship between the sub-scales was studied by means of Spearman's rank correlation coefficient. Results: It was determined that the mean age of the patients was 56.7 ± 14.4, 53% have been diagnosed with chronic renal failure 1 to 5 years ago, and 63% of them have another chronic disease. It was determined that 51% of the patients reported their spirituality and 70% of the patients reported their hope levels as moderate. The mean total Spiritual Orientation Scale score in this study was found to be 99.57 ± 15.51. While the mean total score of Beck Hopelessness Scale sub-scale involving the feelings and expectations about the future was found to be 1.2 ± 1.29, the mean total score of the loss of motivation sub-scale was 2.56 ± 1.89, the mean total score of hope sub-scale was 1.91 ± 1.82 and the mean total score of Beck Hopelessness Scale was found to be 5.68 ± 4.43. A weak negative correlation was found between the Spiritual Orientation Scale and Beck Hopelessness Scale Conclusion: It was determined that as the spiritual orientations of the hemodialysis patients increased, the levels of hopelessness decreased.
