Abstract
ÖZET Bu çalışma, diz OA'li hastalarda aerobik egzersiz ve ev egzersiz programının ağrı ve disabilite üzerine olan etkinliğini göstermek amacıyla yapıldı. Çalışmaya ACR kriterlerine göre diz OA'i tanısı konulan 69 hasta alındı. Hastalar rastgele yöntemle iki gruba ayrıldılar. Birinci gruba 10 dakika ısınma, 40 dakika bisiklet çevirme, 10 dakika soğuma fazlarını içeren 1 saatlik aerobik egzersiz programı haftanın 3 günü uygulandı. İkinci gruba ise kuadriseps güçlendirmeyi amaçlayan ev egzersiz programı önerildi ve 5 egzersizden oluşan bu programı sabah akşam 20 tekrar şeklinde planlandı. Hastalar tedavi öncesi ve sonrasında 12. haftada değerlendirildiler. Tedaviyi 60 hasta tamamladı. Değerlendirmede ağrı için VAS ve WOMAC ağrı skoru, eklem sertliği ve disabilite için WOMAC eklem sertliği, fiziksel fonksiyon skoru, anksiyete ve depresyon için de HAD skorlaması kullanıldı. Tedavi sonrasında her iki grupta VAS ağrı, WOMAC ağrı, fiziksel fonksiyon ve eklem sertliği skorlarında çok anlamlı düzelme görüldü (p<0.001). Anksiyete ve depresyon skorlarındaki iyileşme de istatistiksel olarak anlamlıydı (p<0.01). Tedavi sonrasında sadece VAS ağrı skorunda iki grup arasında fark vardı. VAS ağrı skoru aerobik egzersiz grubunda, ev egzersiz grubuna göre istatistiksel olarak daha düşüktü (sırasıyla, 2.43 ± 0.34, 3.60 ± 0.33; p<0.05). Diğer parametrelerde iki grup arasında anlamlı fark yoktu (p>0.05). Aerobik egzersiz ve kuadriseps güçlendirme programının diz OA'li hastalarda ağrı, disabilite, eklem sertliği, anksiyete ve depresyonda anlamlı düzelme sağladığı ve mutlaka tedavinin bir parçası olması gerektiği sonucuna varıldı. 54
ABSTRACT The aim of this study was to evaluate the effects of an aerobic exercise program versus a home based exercise program designed to improve quadriceps strength, on pain and disability. Sixty nine women between 40 and 67 years old with diagnosed osteoarthritis of one or both knees according to ACR criteria participated in the study. Patients were randomized into two groups. Patients received either an aerobic exercise program performed for one hour (10 minutes warm up, 40 minutes cycling, 10 minutes cool down) 3 days a week, or a home exercise quadriceps strengthening program consisted of 5 exercises ( 20 repetations, twice a day) performed daily. Both groups were treated for 12 weeks. Patients were evaluated before and after the intervention. 60 patients completed the treatment. Outcome measures included VAS and WOMAC pain score for pain, WOMAC physical function and stiffness scores for disability and stiffness, and HAD for anxiety and depression. Both treatment groups showed significant improvements in VAS pain, WOMAC pain, physical function and stiffness scores (pO.OOl). Both of the groups also demonstrated significant improvements in anxiety and depression scores (p<0.01). Except VAS pain score there were no significant differences between the groups (p>0.05). The aerobic exercise group had significantly better VAS pain score than the home exercise group (2.43 ± 0.34 vs 3.60 ± 0.33 respectively; p<0.05). In conclusion, aerobic exercise and a simple program of home quadriceps exercises provided significant improvements in pain, stiffness, disability, anxiety and depression in patients with osteoarthritis of the knee. Exercise should be prescribed as part of the treatment of knee osteoarthritis. 55
