Abstract
Introduction: Obstructive sleep apnea (OSA) is a common syndrome that is characterized by recurrent episodes of partial or complete upper airway obstruction during sleep. To prevent obstruction in the upper airway in this syndrome, various approaches have been used. Oral appliances, especially mandibular advancement devices (MADs) are commonly used as a noninvasive treatment for OSAS. MADs therapy improve the polysomnographic parameters by increasing the airway space. Case Description: Forty-seven and fifty years old male patients who diagnosed with OSA were referred to prosthodonti clinic by an otorhinolaryngologist. In clinical examination, tongue obstruction at the upper airway was observed with main complain frequent and loud snoring. For this reason MADs was planned for treatment. After the lower and upper jaw models according to vertical and horizontal jaw records were taken by positioning the mandible anteriorly. Custom-made monoblock appliance was made in our dental laboratory. Follow up 24 hour later, that patients reported no complaints of symptoms in the temporo-mandibular joints, no pressure on soft tissues. One month after the use of the device, that patients was taken for polysomnographic evaluation again and it was seen that oxyhemoglobin saturation increased and apnea and hypopnea index redused. Discussion: The result of this study showed that if there was an obstruction at the upper airway because of tounge,MADs are effective and well-tolerated treatment choise. However the use of MADs have some complications such as tooth and gingival pain, temporo-mandibular and myofascial discomfort. Conclusion/Clinical significance: MADs is frequently preferred as an alternative method in the treatment of OSA and snoring.
