Abstract
İnvaziv Candida enfeksiyonlarının erken ve doğru tanımlanması ve antifungal duyarlılık testlerinin erken ve doğru olarak verilmesi, morbidite ve mortaliteyi azaltmak için önemli bir değere sahiptir. Kültür sonucunda üreyen Candida türlerinin tanımlanması ve antifungal duyarlılığın çalışılması 3-4 gün sürmektedir. Bu sürenin kısaltılması etkin tedavi açısında önemlidir. Bu nedenle yeni alternatif tanımlama ve antibiyotik duyarlılık testlerine ihtiyaç duyulmaktadır. Çalışmamızda invaziv kandidemi nedeni olan Candida türlerinin tanımlanmasında yeni bir sistem olan MALDI-TOF MS(Matrix-Assisted Laser Desorption Ionization-Time-Of-Flight Mass Spectrometry) ile tanımlanması ve sonuçların konvansiyonel sistemler ile karşılaştırılması, antifungal duyarlılık için yeni bir sistem olan Sensititre Yeastone (Trek Diagnostic System) antifungal duyarlılık kiti ile çalışılıp sonuçların altın standart test olan mikrodilüsyon metoduyla karşılaştırılması amaçlanmıştır. OMÜ Tıp Fak. Hastanesi Tıbbi Mikrobiyoloji Laboratuvarında, değişik servislerde yatan hasta kan örneklerinden izole edilen 71 Candida suşu çalışmaya alındı. Tür tayini germ tüp testi, kromojenik besiyeri ve pirinç ekstresi-Tween 80 agar kullanılarak yapıldı. Tür tayini sonucu göre; 37'si (%52,1) C. albicans, 34'ü (%47,9) non-albicans Candida (20 C. parapsilosis, 7 C. glabrata, 4 C. tropicalis, 2 C. kefyr, 1 C. lusitaniae) olarak tanımlandı. Konvansiyonel yöntem ile tanımlanmış tüm suşların Vitek MS ile de tanımlaması yapıldı ve konvansiyonel yöntemlerle %100 uyumlu bulundu. İzolatların amfoterisin B, kaspofungin, flukonazol, itrakonazol ve vorikonazol'e duyarlılıkları referans yöntem ve Sensititre YeastOne mikroplak yöntemi araştırıldı. Referans mikrodilüsyon yöntemi ile Sensititre YeastOne mikroplak yöntemi arasındaki esansiyel uyum oranları tüm Candida türleri arasında amfoterisin B'de %94,4, flukonazol %92,9, kaspofungin %100, itrakonazol %94,4 vorikonazol'de %98,6 olarak saptanmıştır. Referans mikrodilüsyon yöntemi ile Sensititre YeastOne mikroplak yöntemi arasındaki kategorik uyum oranları kaspofungin için %100, flukonazol için %97,1 vorikonazol için%100 olarak tespit ettik
Early and accurate identification and antifungal susceptibility of invasive candida infections have a significant value in order to reduce morbidity and mortality. After the cultivation, identification and antifungal susceptibility of Candida species lasts for 3-4 days. It is important to shorten the time for the treatment of infections. Therefore, new alternative methods are needed for identification and antibiotic susceptibility testing. In this study, it is aimed to compare MALDI-TOF MS (Matrix-Assisted Laser Desorption Ionization-Time-Of-Flight Mass Spectrometry) which is a new method in identification of Candida spp. and conventional methods in identication of Candida spp. from invasive infections and furthermore to compare Sensititre Yeastone (Trek Diagnostic System) a new method for testing antifungal susceptibility with microdilution method which is a gold standart for determination of antifungal susceptibility. Seventy-one Candida strains isolated from blood samples of the patients staying in different services at Ondokuz Mayıs University, Medical Faculty Hospital Medical Microbiology Laboratory were included in the study. Species identification germ tube test was made using chromogenic medium and brass extract-Tween 80 agar. According to species identification result, 37 of the species (52.1%) were identified as C. albicans and 34 of them (47.9%) were identified as non-albicans Candida (20 C. parapsilosis, 7 C. glabrata, 4 C. tropicalis, 2 C. kefyr, 1 C. lusitaniae). All conventionally identified strains were also identified with Vitek MS and they were 100% compatible with conventional methods. Sensitivity of isolates to amphotericin B, caspofungin, fluconazole, itraconazole and voriconazole was examined with reference method and Sensititre YeastOne microplate method. Essential agreement ratios between the reference microdilution method and the Sensititre YeastOne microplate method were detected as 94.4% in amphotericin B, 92.9% in fluconazole, 100% in caspofungin, 94.4% in itraconazole, and 98.6% in voriconazole among all Candida species. Categorical agreement ratios between reference microdilution method and Sensititre YeastOne mikroplate method were detected as 100% for caspofungin, 97.1% for fluconazole, and 100% for voriconazole.
