Gingivitisli ve Periodontitisli Bireylerde YKL-40 Akut Faz Proteini ve İnterlökin-6 Seviyelerinin Biyokimyasal Olarak İncelenmesi

Abstract

Amaç: Yeni bir akut faz proteini olan YKL-40'ın romatoid artrit, tip 2 diabet ve koroner arter hastalıkları gibi inflamatuar hastalıklarda arttığı gösterilmiştir. Ancak, YKL-40 ve periodontal hastalık arasındaki ilişkiyi inceleyen bir çalışma bulunmamaktadır. İnterlökin-6 (IL-6); akut faz proteini sentezinin majör düzenleyicisi olup, periodontal hastalıklarda en fazla çalışılmış inflamatuar belirleyicilerden biridir. Bu çalışmada, sağlıklı bireyler ile periodontal hastalığı olan bireylerde dişeti oluğu sıvısı (DOS) ve serumda YKL-40 ve IL-6 seviyelerinin değerlendirilmesi amaçlandı. Materyal ve Metot: Sistemik hastalığı olmayan periodontal olarak sağlıklı (n:40), gingivitisli (n:40) ve şiddetli kronik periodontitisli (n:40) bireyler çalışmaya dahil edildi. Klinik ölçümler kaydedildi ve her bireyden DOS ve kan örnekleri toplandı. DOS ve serum YKL-40 ve IL-6 seviyeleri ELISA ile analiz edildi. İstatistiksel değerlendirme parametrik ve nonparametrik testler kullanılarak yapıldı. Bulgular: DOS YKL-40 ve IL-6 total miktarı ile serum YKL-40 ve IL-6 seviyeleri gingivitis ve kronik periodontitis hastalarında sağlıklı kontrol grubuna göre anlamlı olarak daha yüksekti (P<0,001). DOS ve serum YKL-40 ve serum IL-6 seviyelerinde kronik periodontitis hastalarında gingivitis hastaları ile karşılaştırıldığında istatistiksel anlamlı artış saptandı (P<0,001). Sonuç: DOS YKL-40, serum YKL-40 ve IL-6 seviyeleri gingivitisten periodontitise artış gösterdi. Bu çalışmanın sınırları dahilinde, YKL-40 molekülü periodontal hastalıkların potansiyel yeni bir inflamasyon belirleyicisi olabilir. Aim: YKL-40, a new acute phase protein, is shown to be elevated in inflammatory diseases such as rheumatoid arthritis, type 2 diabetes mellitus and coronary artery diseases. However, there is no data indicating relationship between YKL-40 and periodontal disease. Interleukin-6 (IL-6) is the major regulator of acute phase protein synthesis and one of the most studied inflammatory markers in periodontal disease. The purpose of the present study was to evaluate YKL-40 and IL-6 levels in gingival crevicular fluid (GCF) and serum of patients with periodontal disease and healthy subjects. Material and Method: Periodontal healthy subjects (n:40) and patients with gingivitis (n:40) and severe chronic periodontitis (n:40) without any systemic disease were included to the study. Clinical measurements were recorded; GCF and blood samples were obtained from each subject. GCF and serum YKL-40 and IL-6 levels were analyzed by using ELISA. Statistical analysis was performed by parametric and non-parametric tests. Results: Total amounts of YKL-40 and IL-6 in GCF, and serum YKL-40 and IL-6 levels were significantly higher in gingivitis and chronic periodontitis patients compared to healthy controls (P<0.001). YKL-40 levels in GCF and serum, and serum IL-6 levels were significantly higher in chronic periodontitis patients compared to gingivitis patients (P<0.001). Conclusion: YKL-40 levels in GCF, and serum YKL-40 and IL-6 levels increased from gingivitis to periodontitis. Within the limits of the present study, YKL-40 molecule might be a potential novel inflammatory marker of periodontal disease.
